Of course we have paperwork for you to fill out for the first appointment. You can either come 15 minutes early and do them, or you can summit them to us electronically with the links below, or print out the PDF version and bring it with you. All therapists except Ishatova use the Client Registration Form. All use the Billing Form. Please come a bit early even if you use these forms.

The electronic submissions are encrypted for security. If you would rather not use the online form, please print out a copy and have it filled out upon arrival.
